Financial Management Services (FMS)?
Financial Management Services (FMS) are for people who want to self-direct their services through Consumer Directed Community Supports (CDCS) or the Consumer Support Grant (CSG). CDCS and CSG programs give people flexibility and control over their service planning and supports. They help people live independently in their homes and communities.
Best Care FMS assists with budget management and supports participants in their role as the employer of their workers. Participants often use CDCS or CSG to purchase goods and services and hire family and friends. Participants directly employ their workers, and as the employer, have the ability to set pay rates for their employees and offer other employee benefits.
Community First Services + Supports
What is CFSS?
The Minnesota Department of Human Services is preparing to transition from personal care assistance (PCA) to community first services and supports (CFSS). Both PCA and CFSS provide supports to people to help them remain independent in the community and are similar in many ways.
Additionally, CFSS will expand people’s choices about how they receive their services, including who can provide services, additional support for writing plans, more self-direction options and the ability to
purchase goods to aid a person’s
independence.
245D Waivered Services
Accra is a 245D licensed provider of respite, homemaker, Individual Community Living Support (ICLS) services, Individual Home Supports (IHS) without training, IHS with training, and night supervision services. For each 245D service, clients select a trusted, qualified worker suited for their needs, while for IHS with training, a qualified staff is matched to the client to meet their needs. In all services, an assigned 245D Accra Service Coordinator will work with the client and county case manager to set up and administer 245D Waivered Services.
Accra also administers Caregiver Living Expenses, which provide a reimbursement of certain expenses, such as rent and food, incurred by an unrelated caregiver who is living in the same household as an individual
receiving waivered services and providing approved services.
