Financial Management Services (FMS)?
Financial Management Services (FMS) are for people who want to self-direct their services through Consumer Directed Community Supports (CDCS) or the Consumer Support Grant (CSG). CDCS and CSG programs give people flexibility and control over their service planning and supports. They help people live independently in their homes and communities.
A Heart Of Joy Home Health LLC Assist with FMS with budget management and supports participants in their role as the employer of their workers. Participants often use CDCS or CSG to purchase goods and services and hire family and friends. Participants directly employ their workers, and as the employer, have the ability to set pay rates for their employees and offer other employee benefits.
Community First Services + Supports
What is CFSS?
The Minnesota Department of Human Services is preparing to transition from personal care assistance (PCA) to community first services and supports (CFSS). Both PCA and CFSS provide supports to people to help them remain independent in the community and are similar in many ways.
Additionally, CFSS will expand people’s choices about how they receive their services, including who can provide services, additional support for writing plans, more self-direction options and the ability to
purchase goods to aid a person’s
independence.
245D Waivered Services
A Heart Of Joy Home Health LLC holds a 245D license and offers a range of essential services, including respite care, homemaker assistance, Individual Community Living Support (ICLS), Individual Home Supports (IHS) with and without training, as well as night supervision services. Clients have the freedom to choose a qualified and trustworthy worker for each 245D service. When it comes to IHS with training, our team ensures that a suitable and skilled staff member is matched with the client to meet their specific needs. Across all our services, a dedicated 245D Accra Service Coordinator collaborates with the client and county case manager to establish and oversee the provision of 245D Waivered Services.
Moreover, A Heart Of Joy Home Health LLC also manages Caregiver Living Expenses, which involves reimbursing eligible costs such as rent and food incurred by an unrelated caregiver residing in the same household as the individual receiving waivered services while providing approved care.
